The rear cap sits on the bayonet of your manual Mamiya 645 lenses and keeps dust and debris away from the rear element during transport. It replaces the original cap, which is almost always missing on lenses bought used.
The AF generation is built differently at the rear: this cap does not fit Mamiya 645 AF and AFD lenses or Phase One 645 lenses.
